"Acting on SDOH Screening Data to Address Barriers to Health " Webinar
Health centers provide high-quality primary and preventive care to diverse populations in special and vulnerable populations in underserved communities. Health centers also provide a host of enabling services – non-clinical services that aim to increase access to health care and improve population health outcomes. The ability to track and evaluate these services is an important tool for health centers to demonstrate their value in addressing patients’ social determinants of health; of equal importance is the ability to make this data actionable in order to address patients’ barriers to access.

The Association of Asian Pacific Community Health Organizations, Health Outreach Partners, MHP Salud, and National Health Care for the Homeless Council invite you to our webinar to explore strategies to screen special and vulnerable populations for social determinants of health (SDOH) and build effective practices to begin addressing SDOH through outreach and enabling services (ES).

Learning Objectives:
(1) Identify Enabling Services (ES) workforce providers for SDOH screening of special and vulnerable populations at health centers.
(2) Show the value of ES staff and how they can impact clinical and non-clinical decision-making at health centers.
(3) Demonstrate the impact of analyzing SDOH screening data and increasing internal and external capacity to address patient social needs.

Primary Audience: A HRSA-funded Health Centers and Health Center Program Look-Alike staff from both clinical and non-clinical perspectives and from all levels of leadership. PCAs and HCCNs are welcome to participate in all sessions as you continue to support health centers in your state or region.
